Health Office Medication Procedures
APS staff are no longer able to fax medical providers regarding orders to provide nursing care for your student. It is critical that you obtain these orders, signed by the provider, at least a week PRIOR to the beginning of the in-classroom instruction.
Medications will not be accepted until that time.
All current health forms can be obtained from the health office or at APS nursing .
Please bring any medications, supplies, or emergency medications with completed order forms to meet the teacher day or the first day of school.
No medication or supplies can be accepted without a valid order signed by the medical provider and the parent/guardian.

Health Screenings
All Kindergarteners, 1st Graders, and 3rd Graders will have state or district mandated health screenings performed as well as all new students from outside of APS. If any problems are noted, the school nurse will notify you. If at any time you have concerns about your child's hearing, vision, growth, or dental health please contact the health office.
